Intestinal barrier dysfunction plays a critical role in perioperative complications, especially in the circumstances where immunologic barrier is damaged. This paper reviews the structure of the intestinal immunologic barrier, its interaction with intestinal microbiota, and its effects on intestinal epithelial cells. It emphasizes the key role of the intestinal immunologic barrier in maintaining intestinal homeostasis and explains how its dysfunction increases the risk of infection, anastomotic leakage, gastrointestinal motility disorders, and systemic inflammatory response syndrome during the perioperative period.
